360 Balance & Dizziness is happy to accept the following insurances:
- We are a Medicare and BCBS participating provider for vestibular diagnostics and physical therapy services
- We accept United Health Care for physical therapy but not vestibular diagnostics
- We accept Cigna for vestibular diagnostics but not physical therapy
We no longer accept Humana or Aetna for physical therapy or vestibular diagnostics.
Please note that although you can see our clinicians without a referral by law, most insurance companies require a referral to reimburse for our services. If you are private pay, you can schedule a visit without a referral but will need a referral for any visits after 10 days. A referral is a medical order from your doctor that must read “evaluate & treat.”
Vestibular diagnostics include:
- Videonystagmography (VNG)
- Acoustic Brainstem Response (ABR)
- Electrocochleography (ECoG)
- Vestibular Evoked Myogenic Potential (VEMP)
- Computerized Dynamic Visual Acuity (DVAT)
- Computerized Dynamic Posturography (CDP)
